The key mechanism of action of the risk gene APOE4, carried by up to 75% of patients with Alzheimer disease and 25% of the global population, has been identified. Contrary to the prevailing theory, APOE4 produced within neurons was found to cause neuronal atrophy and excessive excitability by overexpressing the Nell2 gene. In an animal study, suppressing Nell2 expression restored early neuronal damage, opening the possibility of developing new treatments.
