Resistance to PD-1 inhibitors in esophageal squamous cell carcinoma is associated with reduced MHC I antigen presentation, with loss of the transcription factor KLF4 identified as a key cause. KLF4 activates MHC I gene expression by maintaining chromatin accessibility and assembling the enhanceosome, and this mechanism is impaired in more than half of patients. Reinducing KLF4 with APT-253 restored the tumors’ sensitivity to immunotherapy.
